We will make every effort to collect payment from your insurance company
(as
we are expected to do according to our contract);
at the same time, we expect that you will facilitate this [often
times cumbersome] process by telling us changes in your coverage
or by promptly sending us the appropriate forms.
We respectfully request (and expect),
your copay at the time of visit.
Please
note:We reserve the right to charge you or
your Insurance for missed appointments;particularly, for those not showing
at the time of the scheduled visit, or for last minute cancellation.
If you don't have insurance, we'll discuss with you payment arrangements
at the time of your visit. Please ask to talk to our office manager or
billing staff.
